The doctors, nurses and other staff at Warrengate Medical Centre would like to hear your views on the services we provide and your suggestions of how we can improve things for all patients.
To help us with this, we are setting up a virtual patient representation group so that you can have your say. We will contact you 4 or 5 times a year to alert you to new surveys or to ask your opinion on any changes we are thinking of making. You will also be able to contact us by email with any suggestions of how we could improve our services.
Please note that no medical queries will be responded to and we will not be able to correspond with you about anything personal.
We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
