How to Register

To apply to register as a New Patient at Warrengate Medical Centre you need to contact us for the relevant forms. You will be given a form to complete your Name, Address etc. and also your previous GP details. You will also be given a Health Questionnaire to complete.

Before your registration is complete all patients registering (including any children) will need to attend the surgery with proof of identification and address. Please ask a member of staff for more information on acceptable forms of identification and proof of address.

Once the forms are completed they will be passed to the GPs and you will be informed by post if you have been accepted on to the Practice list.

To register with the practice you must live within the practice area (see next tab).
